Understanding Paralysis

Paralysis occurs when something goes wrong with the way messages pass between your brain and muscles. This can be caused by injury or disease to the nerves that supply the muscles involved in moving that part of the body.

By strict definition paralysis refers to a loss of movement rather than weakened or impaired mobility. The problem specifically begins in the nerve cells leading to the muscles and not the muscle itself.

Paralysis takes on different forms. It can:

o Be complete or partial

o Be temporary or permanent

o Affect a small area (localized)

o Be widespread (generalized)

o Affect one side (unilateral)

o Affect both sides (bilateral)

If the paralysis affects the lower half of the body and both legs it is called paraplegia. If it affects all arms and legs, it is called quadriplegia.

Occasionally you may hear another term used for paralysis. Palsy is paralysis that is accompanied by loss of feeling and uncontrolled tremors. Cerebral palsy, brachial palsy and Bell's palsy are examples.

Causes Of Paralysis

As explained above, with paralysis something prevents your muscles from receiving messages from your brain. The nerves that relay these messages, called neurons, can't function properly, therefore your muscles can't move.

We have two types of neurons - upper and lower. The upper neuron receives the messages from the brain and carries them to the lower neuron. Then the lower neuron signals the muscle.

Even when we are at rest, not actively moving, both neurons are at work to keep our muscles at a normal degree of tightness or tone. If the upper neurons are damaged the muscles become tighter. This increased tightness is called spasticity and we call this spastic paralysis.

On the other hand, if the lower neurons are damaged the muscle tone is decreased and the muscles become loose and floppy. This is known as flaccidity or flaccid paralysis.

This differentiation isn't as cut and dry as it sounds but it does shed some light on why different diseases or injuries can lead to different symptoms and complications. For that reason it's difficult to list all possible causes of paralysis though this report does discuss some of the more prevalent.

In the United States, loss of muscle function most often results from stroke or injury such as a broken neck or back. The loss can be severe and is often considered irreversible. However, our powerful new supplements have the potential for promising results in reversing paralysis and regaining movement.

Some causes of paralysis are discussed below.

Spinal cord injury

According to the National Spinal Cord Statistical Center, every hour of every day someone is spinal cord injured.

The spinal cord is the main pathway between the brain and the rest of the body. Injury to the spinal cord acts as a roadblock - the messages from your brain can't reach the intended destination - your muscles. Depending on the location and severity of the injury the results can be partial or total paralysis.

Whether through physical injury to the spine or disease of the spine (myelopathy) there is a disturbance of the spinal cord that results in a loss of:

Movement and muscle control

o Patients lose the ability to move their muscles.

o Muscles may waste away due to degeneration of cells and have spastic contractions.

Sensation

o Patients are often unable to feel pain, pressure or hot and cold temperatures.

o Their body can no longer regulate its internal temperature.

Organ System control

o Patients lose control of their bowels and bladder.

o They can't regulate their heart rate or blood pressure.

o They need help with chronic pain, and often sexual function or fertility is diminished.

These losses occur whether the spinal cord is completely severed or is damaged, yet still intact.

In a complete injury, movement and sensation below the level of the injury is impossible. Both sides of the body are affected equally. Someone with an incomplete injury may retain some movement and sensation below the level of injury.

Spinal cord injuries not only result from trauma as described above, but also from diseases such as polio (though extremely rare in the United States), spina bifida and tumors.

Stroke

A stroke occurs when the blood supply to part of the brain is suddenly interrupted or when a blood vessel in the brain bursts, spilling blood into the spaces surrounding brain cells. Brain cells die when they no longer receive oxygen and nutrients from the blood or there is sudden bleeding into or around the brain.

The symptoms of a stroke include sudden numbness or weakness, especially on one side of the body; sudden confusion or trouble speaking or understanding speech; sudden trouble seeing in one or both eyes; sudden trouble with walking, dizziness, or loss of balance or coordination; or sudden severe headache with no known cause.

There are two forms of stroke: ischemic - blockage of a blood vessel supplying the brain, and hemorrhagic - bleeding into or around the brain.

Although stroke is a disease of the brain, it can affect the entire body. A common disability that results from stroke is complete paralysis on one side of the body, called hemiplegia. A related disability that is not as debilitating as paralysis is one-sided weakness or hemiparesis.

Recurrent stroke is frequent; about 25 percent of people who recover from their first stroke will have another stroke within 5 years.

Amyotrophic lateral sclerosis (ALS)

Amyotrophic lateral sclerosis (ALS), sometimes called Lou Gehrig's disease, is a rapidly progressive, deadly neurological disease that attacks the nerve cells (neurons) responsible for controlling voluntary muscles. The disease belongs to a group of disorders known as motor neuron diseases, which are characterized by the gradual degeneration and death of motor neurons.

Motor neurons are nerve cells located in the brain, brainstem, and spinal cord that serve as controlling units and vital communication links between the nervous system and the voluntary muscles of the body. Messages from motor neurons in the brain (called upper motor neurons) are transmitted to motor neurons in the spinal cord (called lower motor neurons) and from them to particular muscles. In ALS, both the upper motor neurons and the lower motor neurons degenerate or die, ceasing to send messages to muscles. Unable to function, the muscles gradually weaken, waste away (atrophy), and twitch (fasciculations). Eventually, the ability of the brain to start and control voluntary movement is lost.

ALS causes weakness with a wide range of disabilities. Eventually, all muscles under voluntary control are affected, and patients lose their strength and the ability to move their arms, legs, and body. When muscles in the diaphragm and chest wall fail, patients lose the ability to breathe without ventilatory support. Most people with ALS die from respiratory failure, usually within 3 to 5 years from the onset of symptoms. However, about 10 percent of ALS patients survive for 10 or more years.

ALS does not affect a person's ability to see, smell, taste, hear, or recognize touch. Patients usually maintain control of eye muscles and bladder and bowel functions, although in the late stages of the disease most patients will need help getting to and from the bathroom.

As many as 20,000 Americans have ALS, and an estimated 5,000 people in the United States are diagnosed with the disease each year. ALS is one of the most common neuromuscular diseases worldwide, and people of all races and ethnic backgrounds are affected. ALS most commonly strikes people between 40 and 60 years of age, but younger and older people also can develop the disease. Men are affected more often than women.

In 90 to 95 percent of all ALS cases, the disease occurs apparently at random with no clearly associated risk factors. Patients do not have a family history of the disease, and their family members are not considered to be at increased risk for developing ALS.

About 5 to 10 percent of all ALS cases are inherited. The familial form of ALS usually results from a pattern of inheritance that requires only one parent to carry the gene responsible for the disease.

Autoimmune responses-which occur when the body's immune system attacks normal cells-have been suggested as one possible cause for motor neuron degeneration in ALS. Some scientists theorize that antibodies may directly or indirectly impair the function of motor neurons, interfering with the transmission of signals between the brain and muscles.

In searching for the cause of ALS, researchers have also studied environmental factors such as exposure to toxic or infectious agents. Other research has examined the possible role of dietary deficiency or trauma. However, as of yet, there is insufficient evidence to implicate these factors as causes of ALS.

Transverse myelitis

Transverse myelitis is a neurological disorder caused by inflammation across both sides of one level, or segment, of the spinal cord. The term myelitis refers to inflammation of the spinal cord; transverse simply describes the position of the inflammation, that is, across the width of the spinal cord. Attacks of inflammation can damage or destroy myelin, the fatty insulating substance that covers nerve cell fibers. This damage causes nervous system scars that interrupt communications between the nerves in the spinal cord and the rest of the body.

Symptoms of transverse myelitis include a loss of spinal cord function over several hours to several weeks. What usually begins as a sudden onset of lower back pain, muscle weakness, or abnormal sensations in the toes and feet can rapidly progress to more severe symptoms, including paralysis, urinary retention, and loss of bowel control. Although some patients recover from transverse myelitis with minor or no residual problems, others suffer permanent impairments that affect their ability to perform ordinary tasks of daily living. Most patients will have only one episode of transverse myelitis; a small percentage may have a recurrence.

The segment of the spinal cord at which the damage occurs determines which parts of the body are affected. Nerves in the cervical (neck) region control signals to the neck, arms, hands, and muscles of breathing (the diaphragm). Nerves in the thoracic (upper back) region relay signals to the torso and some parts of the arms. Nerves at the lumbar (mid-back) level control signals to the hips and legs. Finally, sacral nerves, located within the lowest segment of the spinal cord, relay signals to the groin, toes, and some parts of the legs.

Researchers are uncertain of the exact causes of transverse myelitis. The inflammation that causes such extensive damage to nerve fibers of the spinal cord may result from viral infections, abnormal immune reactions, or insufficient blood flow through the blood vessels located in the spinal cord. Transverse myelitis also may occur as a complication of syphilis, measles, Lyme disease, and some vaccinations, including those for chickenpox and rabies. Cases in which a cause cannot be identified are called idiopathic.

How Does This Happen?

Transverse myelitis often develops following viral infections. Infectious agents suspected of causing transverse myelitis include varicella zoster (the virus that causes chickenpox and shingles), herpes simplex, cytomegalovirus, Epstein-Barr, influenza, echovirus, human immunodeficiency virus (HIV), hepatitis A, and rubella. Bacterial skin infections, middle-ear infections (otitis media), and Mycoplasma pneumoniae (bacterial pneumonia) have also been associated with the condition.

In post-infectious cases of transverse myelitis, immune system mechanisms, rather than active viral or bacterial infections, appear to play an important role in causing damage to spinal nerves. Although researchers have not yet identified the precise mechanisms of spinal cord injury in these cases, stimulation of the immune system in response to infection indicates that an autoimmune reaction may be responsible. In autoimmune diseases, the immune system, which normally protects the body from foreign organisms, mistakenly attacks the body's own tissue, causing inflammation and, in some cases, damage to myelin within the spinal cord.

An acute, rapidly progressing form of transverse myelitis sometimes signals the first attack of multiple sclerosis (MS), however, studies indicate that most people who develop transverse myelitis do not go on to develop MS. Patients with transverse myelitis should nonetheless be screened for MS because patients with this diagnosis will require different treatments.

Transverse myelitis may be either acute (developing over hours to several days) or subacute (developing over 1 to 2 weeks). Initial symptoms usually include localized lower back pain, sudden paresthesias (abnormal sensations such as burning, tickling, pricking, or tingling) in the legs, sensory loss, and paraparesis (partial paralysis of the legs). Paraparesis often progresses to paraplegia (paralysis of the legs and lower part of the trunk). Urinary bladder and bowel dysfunction is common. Many patients also report experiencing muscle spasms, a general feeling of discomfort, headache, fever, and loss of appetite. Depending on which segment of the spinal cord is involved, some patients may experience respiratory problems as well.

Progression of the disease over several weeks often leads to full paralysis of the legs, requiring the patient to use a wheelchair.

Guillain-Barre syndrome

Guillain-Barré syndrome is a disorder in which the body's immune system attacks part of the peripheral nervous system. The first symptoms of this disorder include varying degrees of weakness or tingling sensations in the legs. In many instances, the weakness and abnormal sensations spread to the arms and upper body.

These symptoms can increase in intensity until the muscles cannot be used at all and the patient is almost totally paralyzed. In these cases, the disorder is life threatening and is considered a medical emergency. The patient is often put on a respirator to assist with breathing.

Most patients, however, recover from even the most severe cases of Guillain-Barré syndrome, although some continue to have some degree of weakness. Guillain-Barré syndrome is rare. Usually Guillain-Barré occurs a few days or weeks after the patient has had symptoms of a respiratory or gastrointestinal viral infection. Occasionally, surgery or vaccinations will trigger the syndrome. The disorder can develop over the course of hours or days, or it may take up to 3 to 4 weeks.

No one yet knows why Guillain-Barré strikes some people and not others or what sets the disease in motion. What scientists do know is that the body's immune system begins to attack the body itself, causing what is known as an autoimmune disease. Guillain-Barré is called a syndrome rather than a disease because it is not clear that a specific disease-causing agent is involved. Because the signals traveling along the nerve are slower, a nerve conduction velocity (NCV) test can give a doctor clues to aid the diagnosis. The cerebrospinal fluid that bathes the spinal cord and brain contains more protein than usual, so a physician may decide to perform a spinal tap.

Guillain-Barré syndrome can be a devastating disorder because of its sudden and unexpected onset. Most people reach the stage of greatest weakness within the first 2 weeks after symptoms appear, and by the third week of the illness 90 percent of all patients are at their weakest. The recovery period may be as little as a few weeks or as long as a few years. About 30 percent of those with Guillain-Barré still have a residual weakness after 3 years. About 3 percent may suffer a relapse of muscle weakness and tingling sensations many years after the initial attack.

Other conditions have paralysis as a complication such as:
